DESCRIPTION OF EDUCATION, TRAINING AND EXPERIENCE
Ph.D. in Clinical Psychology, Graduated from BYU in 12/2006 Licensed Psychologist I specialize in psychological assessment. I have completed hundreds of evaluations since 2000 and can complete them quickly and efficiently, yet thoroughly. We have other providers in our offices too, who specialize in therapy and assessments. We also have psychiatrists at our Salt Lake office.
SERVICES AND TRAVEL FEES
We generally charge $100 per hour for assessment services and $85 per session for therapy.
$0.40 per mile if the appointment is farther than 20 miles from our office.
AREAS SERVED
Statewide, but I generally like to have at least 2 students in an area if I have to travel more than an hour.
SCHEDULE OF SERVICES
Mondays through Fridays 9-6:00...I can also work weekends if necessary.
REFERENCES/PREVIOUS CLIENTS
We have contracted for Utah Virtual Academy for the past month. I have done evaluations for Dr. Kenneth Seely at Cross Creek Manor/Academy in Laverkin, Utah I have completed evaluations for Pam McCollam with New Haven in Spanish Fork and their affiliate in Hurricane, Utah. I have completed evaluations for Second Nature Wilderness program in Duchesne and St. George, Utah. I do comprehensive evaluations for DCFS in Salt Lake County.

We also provide EEG Biofeedback/Neuro-feedback services at both our Salt Lake and American Fork offices. |
