SERVICES AND TRAVEL FEES

Cost of initial evaluation $150.00 Includes all necessary paperwork and meeting times.  Individual Treatment: 1 hour $70.00,   30 min $40.00 Group sessions $55.00 Because we have an office, we would request that the client come to our office.  Therefore no mileage would be requested.

AREAS SERVED
We are located in Salt Lake City but we are near several freeway exits.  We have some clients who live down the block and some who come as far as Lehi.

OTHER INFORMATION

We have several standardized assessment tools and a portable audiometer and upon request, we would be willing to conduct just the evaluation or full services depending on the referral/need.   Our office is conveniently located and our therapy staff (Shelly and Mark) are skilled and experienced therapists.  Thank you for considering us as a provider.  We are very interested in working with local charter schools.
